Family Based Therapy Associates (FBTA) welcomes you to its ADHD Clinic services. We believe the information and services offered through our clinic offices will be helpful in assessing and treating the ADHD symptoms you or your child are experiencing. This webpage is designed to inform persons interested in our services about how to contact us, the assessment process and about the FBTA clinicians who provide ADHD related services. The intake forms and some of the necessary assessment instruments can be downloaded from this webpage. Please note that the intake forms and assessment instruments are age specific. Due to copyright laws, other necessary assessment instruments will be mailed to you. ADHD related resource links are included on this webpage.
